- POLICY & PROCEDURES MANUAL (read, initial each page, sign & date last page)
- Registration Form (parent completes)
- Authorization for Emergency Medical Treatment (parent completes)
- Travel and Activity Authorization (parent completes)
- Staff/Volunteer Health Certificate (required for any adult serving duty days in the classroom- completed by an MD, PA or NP)
- Staff/Volunteer Health Information Form (required for any adult serving duty days in the classroom- completed by the adult)
- D.C. Universal Health Certificate (completed by your child's pediatrician)>
- Oral Health Assessment (completed by your child's dentist)
- Anaphylaxis Action Plan (parent completes if applicable)
- Asthma Action Plan (parent completes if applicable)
