This form must reflect a physical within the past 12 months and a tuberculin skin test (PPD) within the past 24 months and must be kept up to date throughout the school year (ie: if you turn in a form reflecting a physical from 11/2012 you will need to turn in another form in 11/2013 which reflects a current physical).
