- Staff/Volunteer Health Certificate (required for any adult serving duty days in the classroom- completed by an MD, PA or NP)
- Staff/Volunteer Health Information Form (required for any adult serving duty days in the classroom- completed by the adult)
- Criminal Background Screening
- D.C. Universal Health Certificate (pediatrician completes)>
- Oral Health Assessment (dentist completes)
- Anaphylaxis Action Plan (pediatrician completes if applicable)
- Asthma Action Plan (pediatrician completes if applicable)
