1. Universal Health Certificate
*You should also take the Universal Health Certificate Instructions to your child's pediatrician
*You should also take the Immunization Schedule with you to your child's check-up to ensure he/she has had all
required immunizations.
2. Staff/Volunteer Health Certificate
3. Oral Health Assessment Form
4. Anaphylaxis Action Plan (if applicable)
5. Asthma Action Plan (if applicable)
CLEARANCES TO OBTAIN FROM AN OUTSIDE AGENCY
